Job Description
Take on an impactful nursing role supporting patients in a home health care setting. This position is an 8-week contract with the potential to extend to 12 weeks, focused on providing expert care and case management across the Greensboro and High Point areas. The role requires an energetic registered nurse who is confident navigating community-based settings and skilled in managing diverse patient needs.
Schedule:
- Monday to Friday
- 8:00 AM 5:00 PM
- Day shift, 5x8-hour shifts per week
Required Experience and Qualifications:
- Active RN license
- Minimum of 1 year of case management experience
- Previous home health experience
- Prior HCHB (Homecare Homebase) experience
- Travel nursing experience preferred
- Must have reliable transportation and a valid drivers license
Key Responsibilities:
- Manage a caseload of home health patients, conducting in-home visits and assessments
- Develop and implement individualized care plans
- Educate patients and their families on treatment plans, self-care, and disease management
- Coordinate with interdisciplinary healthcare teams to optimize patient outcomes
- Accurately document all care activities and patient interactions using HCHB
- Travel between patient homes throughout each workday (mileage reimbursed at the federal rate)
