This contract opportunity involves providing skilled nursing services in a home health setting, covering the Greensboro and High Point areas. Collaborate as part of a dedicated team that values patient-centered care, professionalism, and autonomy.

Schedule/Hours:

• Monday through Friday, 8:00 am 5:00 pm
• Day shift, 5x8-hour shifts
• 8-week assignment with potential extension up to 12 weeks, depending on census

Qualifications:

• Active and unrestricted RN license
• Minimum 1 year of experience in home health case management
• Demonstrated experience managing and maintaining a caseload
• Previous home health and travel nursing experience required
• Reliable personal vehicle required for daily travel (mileage reimbursement provided at federal rate)
• Strong organizational, communication, and documentation skills
• Proficiency with electronic health records and home health documentation systems
• Flexible and adaptable, comfortable working independently across multiple locations throughout the day

Key Responsibilities:

• Provide skilled nursing care and case management to patients in their homes
• Develop, implement, and monitor individualized care plans
• Collaborate with physicians, therapists, and other healthcare professionals
• Conduct patient assessments, administer medication, and monitor progress
• Educate patients and families on health management and preventive care
• Maintain accurate and timely documentation in alignment with regulatory and agency requirements
