An exciting opportunity is available for an experienced Registered Nurse skilled in home health care. This contract role spans an initial eight weeks, with the potential to extend to twelve weeks, and focuses on providing top-tier care to patients in their homes. You'll cover the Greensboro and High Point areas, managing your own caseload and playing a crucial part in supporting the health and independence of clients outside the inpatient setting.

Schedule/Hours:

• Monday through Friday
• 8:00 am to 5:00 pm
• Day shift, 5x8-hour schedule

Qualifications:

• Active RN license required
• At least one year of case management experience
• Prior experience in home health care, including HCHB and travel nursing
• Reliable transportation and willingness to travel between patients throughout the day
• Strong organizational and communication skills
• Ability to work independently and prioritize patient needs

Key Responsibilities:

• Assess patient needs and develop individualized care plans
• Provide nursing treatments and patient education in the home setting
• Case manage a dedicated caseload, coordinating care and resources
• Document care and interventions accurately and promptly
• Communicate effectively with interdisciplinary teams, patients, and families
• Maintain compliance with all home health protocols and regulations
