Take on a rewarding role supporting patients in their homes, ensuring quality care and positive outcomes as a Registered Nurse specializing in home health. This full-time, 13-week contract position is designed for experienced RNs seeking to make a tangible difference through field-based care.

Schedule and Setting:

• Monday Friday, 8:00 AM 5:00 PM
• Field-based role providing daily visits to patient homes within an assigned territory

Responsibilities:

• Complete thorough OASIS assessments and develop individualized nursing care plans
• Deliver skilled nursing services such as wound care, medication administration, IV therapy, and chronic disease management
• Monitor patient progress, assess for changes in condition, and promptly update care teams and physicians
• Offer patient and family education on conditions, treatments, medications, and preventive care measures
• Document visits and care in Home Care Home Base (HCHB) system accurately and promptly
• Collaborate with an interdisciplinary team, including therapists, social workers, and aides, to provide holistic care
• Attend case conferences and staff meetings; engage in ongoing professional development
• Ensure compliance with regulatory and agency standards in all patient care and documentation

Qualifications:

• Active RN license in good standing (state of practice)
• At least one year of recent home health nursing experience; field experience strongly preferred
• Proficiency in both HCHB and OASIS required
• Valid drivers license, reliable transportation, and current auto insurance

$2,057 - $2,176/Week Estimated weekly earnings represent hourly wages, based on 40 hours per week, and expense reimbursement for eligible candidates. The actual amount may vary based on hours worked, eligibility for reimbursements, and seasonal adjustments.
