Your next opportunity to make a difference in patient lives starts here. We are seeking a skilled Registered Nurse (RN) with strong OASIS experience for a full-time 13-week home health contract. This role requires confident clinical ability, accuracy in documentation, and excellent time management. Deliver care that truly mattersright in the patients own home.
Schedule:
- 13-week assignment, with potential to extend
- Full-time hours, Monday through Friday
- On-call rotation is probable
Qualifications:
- Active RN license
- At least one year of recent experience as a home health RN
- Solid OASIS documentation skills (required)
- Proficiency in HomeCare HomeBase is strongly preferred
- Ability to organize and maintain a full visit schedule daily
- Reliable transportation and valid drivers license
Primary Responsibilities:
- Provide skilled nursing interventions and in-home patient care
- Conduct initial and ongoing patient assessments, completing precise OASIS documentation
- Collaborate with physicians and care teams to shape effective care plans
- Carry out wound care, medication management, disease monitoring, and patient/family education
- Communicate clearly regarding changes in condition to appropriate team members
