Take your nursing expertise on the road and make a meaningful impact by delivering compassionate care to patients in their homes. This full-time, 13-week contract position offers a balanced Monday through Friday schedule (8:00 AM 5:00 PM) and centers on comprehensive patient care within a supportive environment.

Schedule:

• Monday Friday, 8:00 AM 5:00 PM

Role highlights:

• Expect a weekly caseload of 30 points (approximately 35 patient visits per day)
• Field-based care, providing vital nursing services in patients' homes within your assigned territory
• Mileage reimbursement for daily travel

Key responsibilities:

• Complete thorough OASIS assessments and develop personalized care plans
• Deliver skilled nursing care, including wound care, medication administration, IV therapy, and chronic disease management
• Monitor patient conditions, documenting changes and communicating with the care team and physicians as needed
• Educate patients and families on managing illnesses, treatment plans, medications, and preventive health practices
• Accurately and efficiently document in Home Care Home Base (HCHB)
• Collaborate with therapy and social work staff for coordinated, high-quality care
• Participate in staff meetings, case conferences, and educational opportunities
• Ensure compliance with regulatory and agency standards for documentation and patient care

Qualifications:

• Active RN license in the state of practice, in good standing
• At least 1 year of recent field Home Health experience
• Proficient in Home Care Home Base (HCHB) and experienced with OASIS
• Reliable transportation, valid drivers license, and current auto insurance

$2,057 - $2,176/Week Estimated weekly earnings represent hourly wages, based on 40 hours per week, and expense reimbursement for eligible candidates. The actual amount may vary based on hours worked, eligibility for reimbursements, and seasonal adjustments.
