Your next nursing opportunity starts here. Were seeking a dedicated Registered Nurse (RN) skilled in home health care and OASIS documentation for a 13-week full-time contract. This role is perfect for confident clinicians committed to delivering high-quality, individualized patient care in the home setting.
Schedule:
- Full-time hours, Monday through Friday
- On-call responsibilities may apply
- Structured daily visit expectations
Key Responsibilities:
- Deliver high-standard, compassionate in-home nursing care and skilled interventions
- Conduct thorough patient assessments and complete precise OASIS documentation to ensure compliance and timely reporting
- Collaborate with physicians and multidisciplinary care teams to develop and implement individualized care plans
- Provide wound care, medication management, patient and family education, and disease monitoring
- Communicate notable patient updates and evolving needs to the clinical team
Qualifications:
- Active RN license
- Minimum one year of recent experience in home health (required)
- Proficient in OASIS documentation (required)
- Experience with HomeCare HomeBase (strongly preferred)
- Proven ability to manage a structured, full-time schedule with daily patient visits
- Valid drivers license and reliable transportation
