We are seeking a dedicated Registered Nurse with strong home health experienceespecially with OASIS documentationto support patients for a 13-week full-time contract. This is an excellent chance for a skilled clinician who values organization, accuracy, and meaningful patient interactions.
Schedule:
- Full-time, Monday through Friday
- On-call responsibilities are probable
- Structured daily visit requirements
Key Responsibilities:
- Deliver in-home nursing care and skilled interventions consistent with physician-ordered care plans
- Complete thorough patient assessments and submit timely, precise OASIS documentation
- Perform wound care, medication administration, patient teaching, and disease management
- Communicate critical updates or changes in patient condition to the clinical team promptly
- Collaborate closely with providers and multidisciplinary care teams to optimize patient outcomes
Qualifications:
- Active RN license in good standing
- At least 1 year of recent experience as a home health RN
- Demonstrated proficiency with OASIS is required
- Familiarity with HomeCare HomeBase highly preferred
- Proven ability to manage a structured, consistent full-time schedule
- Reliable transportation and a valid drivers license
