Schedule/Hours:
- 10-hour day shifts (8:00 AM 6:00 PM)
- Shift patterns: THURS/FRI/SAT/SUN or SAT/SUN/MON/TUES
- Guaranteed 40 hours per week (except during orientation/holidays)
Required Qualifications:
- Valid and unrestricted WI RN license
- BLS certification (American Heart Association)
- 1-2 years of recent Home Health and Hospice nursing experience
- Strong proficiency with Oasis and Epic (Epic required within 12 months)
- Valid driver's license
- Covid-19 vaccination (medical/religious exemptions considered)
- Ability to physically manage frequent driving, stairs, and home environments
Key Responsibilities:
- Deliver admissions, death visits, symptom management, wound and IV care, and chronic disease management
- Maintain timely, comprehensive documentation
- Adapt quickly to evolving schedules and patient needs
- Function independently with a 1:1 patient ratio, primarily with adult patients
- Adhere to infection control protocols and support patient/family education
- Wear plain, well-maintained scrubs as required
