Elevate your nursing practice as a Registered Nurse specializing in Home Health through a dynamic, full-time contract opportunity. This 13-week assignment offers the chance to make a direct impact on patient outcomes by delivering skilled care in a field-based setting.

Schedule & Setting:

• Monday Friday, 8:00 AM 5:00 PM
• Field-based, providing care in patients homes
• Caseload: 30 points per week (typically 35 visits per day)
• Daily travel within an assigned territory (mileage reimbursement provided)

Key Responsibilities:

• Conduct comprehensive OASIS assessments and develop individualized care plans
• Provide skilled nursing services: wound care, medication administration, IV therapy, and chronic disease management
• Evaluate patient status, communicate timely updates to the care team and physicians
• Educate patients and families about disease processes, treatment, medications, and preventive care
• Document care accurately and promptly in Home Care Home Base (HCHB)
• Collaborate with an interdisciplinary team: PT, OT, MSW, and Home Health Aides
• Attend case conferences, staff meetings, and participate in ongoing education
• Maintain compliance with all regulatory and agency documentation standards

Qualifications:

• Active RN license in good standing in the state of practice
• At least one year of recent home health experience, with field experience preferred
• Proficient with Home Care Home Base (HCHB) system
• OASIS experience required
• Reliable transportation, valid drivers license, and auto insurance

$2,057 - $2,176/Week Estimated weekly earnings represent hourly wages, based on 40 hours per week, and expense reimbursement for eligible candidates. The actual amount may vary based on hours worked, eligibility for reimbursements, and seasonal adjustments.
