Your next opportunity in nursing awaits. A full-time Registered Nurse is needed for a 13-week home health contract, providing vital care and support to patients in their own homes. This position places a strong focus on clinical accuracy, effective documentation, and managing daily patient visits, all within a dedicated and professional care environment.
Schedule:
- 13-week assignment, Monday through Friday
- Full-time hours, with possible on-call responsibilities
Core Responsibilities:
- Provide skilled, in-home nursing care to a diverse patient population
- Perform comprehensive assessments and complete OASIS documentation thoroughly and on time
- Coordinate and communicate with physicians and interdisciplinary care teams
- Deliver wound care, manage medications, educate patients and families, and monitor patient conditions
- Accurately relay updates and changes in patient status to the clinical team
Qualifications:
- Active RN license
- At least one year of recent home health nursing experience
- Proven proficiency with OASIS documentation
- Experience with HomeCare HomeBase (preferred)
- Ability to independently manage a structured schedule of daily visits
- Reliable transportation and a valid drivers license
