We are searching for a dedicated Registered Nurse (RN) with a background in home health and strong OASIS documentation skills for a 13-week full-time contract. This is an excellent fit for someone who takes pride in clinical excellence, values accurate charting, and enjoys working with patients in their own homes.
Schedule:
- 13-week assignment with the potential to extend
- Full-time, Monday through Friday
- On-call responsibilities may be required
- Structured daily visit expectations
Key Responsibilities:
- Deliver skilled in-home nursing care, including wound management, medication administration, patient education, and ongoing disease monitoring
- Complete thorough patient assessments and promptly document findings using OASIS
- Collaborate closely with physicians and multidisciplinary care teams to develop and revise individualized care plans
- Clearly communicate patient status updates, concerns, and progress to the clinical team
- Travel between homes using reliable personal transportation
Qualifications:
- Active RN license required
- At least 1 year of recent RN home health experience
- Proven ability to complete accurate OASIS documentation
- Strongly prefer proficiency with HomeCare HomeBase software
- Reliable transportation and a valid drivers license
