Job Description

Provide expert nursing care in a home health setting, focusing on patient assessment, care plan implementation, and education for patients and their families. This role emphasizes delivering quality care aligned with community health standards and organizational policies. Responsibilities include conducting admissions, managing cases, and maintaining detailed electronic records to support interdisciplinary communication.

Qualifications and Experience:

  • Current Registered Nurse (RN) license
  • Minimum of 1 year Certified Home Care experience
  • At least 1 year of Case Management experience
  • Ability to travel to assigned cases as needed
  • Willingness to participate in weekend rotations, approximately once every six weeks

Key Responsibilities:

  • Assess patient conditions in the home care environment
  • Develop and implement individualized care plans
  • Provide ongoing education and support to patients and family members
  • Perform admission assessments in compliance with organizational policies
  • Manage cases following established standards of practice
  • Document patient care and communications accurately and timely using electronic systems

Schedule:

  • Weekend rotations once every six weeks
  • Travel required to patient homes
