Exciting opportunity available for an experienced Registered Nurse with a passion for home health care. Schedule/Hours:
- Weekend rotations are required once every 6 weeks
- Flexibility to work weekends, holidays, and on-call shifts as needed
Qualifications:
- Active RN license in Massachusetts
- Minimum 1 year of certified home care nursing experience
- At least 1 year of direct case management responsibility
- Reliable, independent transportation for home visits
- Willingness to accommodate department rotation requirements (weekends/holidays/on call)
- Strong assessment, documentation, and communication skills
Key Responsibilities:
- Deliver comprehensive clinical care to patients ranging from newborns to seniors in their home settings
- Perform thorough patient assessments and documentation using electronic records
- Implement individualized care plans, monitor progress, and revise as necessary
- Guide patients and families regarding disease management, medication administration, and ongoing home care needs
- Complete admissions and case management responsibilities in accordance with established standards
- Collaborate closely with interdisciplinary teams to provide coordinated, quality care
- Advocate for patient needs and ensure best outcomes through continuous evaluation
