Deliver compassionate, patient-centered nursing care in a home health setting, providing essential services to individuals ranging from newborns to centenarians. Key Responsibilities:
- Conduct thorough assessments of patient conditions in their homes, creating and implementing individualized care plans.
- Provide ongoing education and clinical support to patients and their families to promote health and well-being.
- Manage admissions and coordinate case management activities adhering to strict healthcare standards and documentation protocols.
- Maintain timely and accurate electronic documentation and facilitate interdisciplinary communication.
- Participate in weekend, holiday, and on-call rotations on a scheduled basis (weekend rotations approximately every six weeks).
Qualifications:
- Current RN licensure in the state of Massachusetts.
- Minimum of 1 year certified home care experience.
- Minimum of 1 year case management experience.
- Reliable, independent transportation to perform home visits across the community.
- Willingness to work weekends, holidays, and on-call as per departmental rotation.
