Key Schedule Information:
- Weekend rotation required once every 6 weeks
Qualifications:
- Active RN license required
- Minimum 1 year of certified home care experience
- Minimum 1 year of case management experience
- Proficient in assessing patient needs, developing care plans, and documenting electronically
- Strong ability to educate and support patients and families
- Reliable transportation and ability to travel to assigned cases
Role Responsibilities:
- Conduct thorough patient assessments to identify care needs and coordinate services
- Implement, update, and evaluate individualized care plans in line with best practices
- Lead and participate in admission processes for new patients, completing all required documentation
- Perform case management functions, collaborating closely with interdisciplinary team members
- Educate patients and families on disease management, safety measures, and available resources
- Ensure compliance with organizational philosophy, policies, and community health standards
- Maintain up-to-date and accurate electronic documentation on patient progress and communications
- Participate in weekend rotation every six weeks as scheduled
