Key Schedule Information:

• Weekend rotation required once every 6 weeks

Qualifications:

• Active RN license required
• Minimum 1 year of certified home care experience
• Minimum 1 year of case management experience
• Proficient in assessing patient needs, developing care plans, and documenting electronically
• Strong ability to educate and support patients and families
• Reliable transportation and ability to travel to assigned cases

Role Responsibilities:

• Conduct thorough patient assessments to identify care needs and coordinate services
• Implement, update, and evaluate individualized care plans in line with best practices
• Lead and participate in admission processes for new patients, completing all required documentation
• Perform case management functions, collaborating closely with interdisciplinary team members
• Educate patients and families on disease management, safety measures, and available resources
• Ensure compliance with organizational philosophy, policies, and community health standards
• Maintain up-to-date and accurate electronic documentation on patient progress and communications
• Participate in weekend rotation every six weeks as scheduled
