Ideal candidates are skilled in clinical care, thrive on maintaining strong documentation standards, and are comfortable handling a structured, set schedule within patients homes. Schedule:
- 13-week contract, with the potential for extension
- Full-time, Monday through Friday
- On-call responsibilities likely
Key Responsibilities:
- Deliver in-home nursing care, including wound care and medication management
- Conduct accurate patient assessments with timely OASIS documentation
- Develop and implement individualized care plans in collaboration with physicians and multidisciplinary teams
- Offer disease education, health coaching, and ongoing patient/family support
- Monitor and report changes in patient conditions
- Maintain precise, compliant records and ensure clear communication with the broader clinical team
Qualifications:
- Active RN license
- Minimum 1 year of recent RN experience in home health
- Demonstrated proficiency with OASIS documentation
- Experience with HomeCare HomeBase strongly preferred
- Reliable transportation and valid drivers license
- Excellent time management and interpersonal skills
