Embark on a rewarding opportunity providing skilled nursing care in a field-based home health setting. This full-time, 13-week assignment focuses on delivering compassionate care to patients in their homes, utilizing your expertise to make a significant impact on their health and daily lives.

Schedule:

• Monday through Friday, 8:00 AM 5:00 PM

You will manage a caseload expectation of 30 points weekly, typically equating to 35 patient visits per day within your assigned territory. Reliable daily travel is required, with mileage reimbursement provided.

Key Responsibilities:

• Perform comprehensive OASIS assessments and develop individualized care plans.
• Deliver skilled nursing interventions, including medication administration, IV therapy, wound care, and chronic disease management.
• Monitor patient conditions, update care plans as needed, and communicate promptly with the multidisciplinary team and physicians.
• Educate patients and families on diagnoses, treatment, medication, and self-care strategies.
• Accurately document all care and findings using Home Care Home Base (HCHB).
• Collaborate with physical therapists, occupational therapists, social workers, and home health aides to ensure seamless care coordination.
• Participate in staff meetings, case conferences, and required continuing education.
• Adhere to all professional standards and regulatory requirements for home health care.

Qualifications:

• Active RN license in good standing (state of practice)
• Minimum 1 year of recent home health field experience
• Proficiency in Home Care Home Base (HCHB) and familiarity with OASIS
• Reliable transportation with a valid drivers license and current auto insurance
