This full-time, 13-week contract assignment prioritizes quality and connection, allowing you to make a direct impact on the lives of individuals in need of skilled home health services.

Schedule & Setting:

• Monday Friday, 8:00 AM 5:00 PM
• Field-based, providing care in patient homes
• Caseload: 30 points per week (typically 35 patient visits per day)
• Daily travel required within assigned territory (mileage reimbursement provided)

Key Responsibilities:

• Conduct comprehensive OASIS assessments and develop individualized care plans
• Deliver skilled nursing care, including wound care, IV therapy, medication administration, and chronic disease management
• Closely monitor patient progress and promptly communicate changes with the care team and physicians
• Educate patients and families regarding disease processes, treatment plans, medications, and preventive health
• Accurately document all patient care and communication within Home Care Home Base (HCHB)
• Coordinate closely with physical/occupational therapists, medical social workers, and home health aides
• Actively participate in case conferences, staff meetings, and professional development

Qualifications:

• Active, unencumbered RN license in the state of practice
• At least one year of recent Home Health nursing experience (field experience strongly preferred)
• Proficiency with OASIS assessments and Home Care Home Base (HCHB) documentation system
• Reliable transportation, valid drivers license, and current auto insurance

$2,057 - $2,176/Week Estimated weekly earnings represent hourly wages, based on 40 hours per week, and expense reimbursement for eligible candidates. The actual amount may vary based on hours worked, eligibility for reimbursements, and seasonal adjustments.
