Your next nursing opportunity starts here. We are seeking an experienced Registered Nurse (RN) with proven home health expertise and OASIS proficiency for a rewarding 13-week full-time assignment. This role offers a chance to deliver impactful care in patients homes while collaborating with a supportive and professional team.

Schedule:

• 13-week contract, with the potential for extension
• Full-time, Monday through Friday
• On-call responsibilities may be required

Key Responsibilities:

• Deliver skilled in-home nursing care, including wound care, medication administration, patient education, and disease management
• Perform thorough patient assessments and complete OASIS documentation accurately and promptly
• Develop and implement individualized care plans in coordination with physicians and multidisciplinary care teams
• Effectively communicate any patient status changes to the clinical team
• Manage daily visit expectations and maintain structured schedules

Qualifications:

• Active, unrestricted RN license
• At least 1 year of recent home health RN experience
• Demonstrated OASIS documentation experience (required)
• Proficiency with HomeCare HomeBase (preferred)
• Reliable transportation and a valid drivers license
• Strong organizational skills and the ability to work independently

$2,193 - $2,312/Week Estimated weekly earnings represent hourly wages, based on 40 hours per week, and expense reimbursement for eligible candidates. The actual amount may vary based on hours worked, eligibility for reimbursements, and seasonal adjustments.
