Advance your nursing career by supporting patients in a field-based home health setting. This full-time contract role spans 13 weeks and empowers you to deliver high-quality care to patients in their homes using your expertise in RN-level nursing and home health protocols.

Schedule & Setting:

• Monday Friday, 8:00 AM 5:00 PM
• Field-based: travel daily to patient homes within assigned territory (mileage reimbursement provided)
• Caseload: Average 30 points per week (typically 35 patient visits per day)

Responsibilities:

• Perform thorough OASIS assessments and design individualized care plans.
• Deliver skilled nursing interventions such as wound care, IV therapy, medication administration, and chronic disease management.
• Monitor patient progress, recognizing changes in condition and communicating updates promptly to the interdisciplinary team and physicians.
• Educate patients and families on disease management, medications, and preventive strategies.
• Maintain comprehensive, accurate documentation in Home Care Home Base (HCHB).
• Coordinate closely with physical and occupational therapists, social workers, and home health aides.
• Attend case conferences, staff meetings, and stay current through continuing education.
• Adhere to regulatory and agency standards for both care delivery and documentation.

Qualifications:

• Active RN license in good standing in the state of practice
• At least 1 year of recent home health nursing experience (field experience highly preferred)
• Experience with HCHB electronic system and OASIS assessment tool required
• Dependable transportation, valid drivers license, and current auto insurance

$2,057 - $2,176/Week Estimated weekly earnings represent hourly wages, based on 40 hours per week, and expense reimbursement for eligible candidates. The actual amount may vary based on hours worked, eligibility for reimbursements, and seasonal adjustments.
