We're seeking an experienced Registered Nurse (RN) with OASIS expertise for a 13-week full-time home health contract. If you excel in clinical documentation, patient assessments, and managing a structured schedule, this position allows you to make a significant difference in patients' lives while working with a supportive, professional care team.
Schedule:
- Full-time, Monday through Friday
- On-call responsibilities may be required
- Daily patient visit expectations
Qualifications:
- Active RN license
- Minimum of 1 year recent experience as a home health RN
- Demonstrated OASIS documentation skills (required)
- Experience with HomeCare HomeBase (strongly preferred)
- Reliable transportation and a valid driver's license
- Ability to autonomously manage a full-time visit schedule
Key Responsibilities:
- Deliver high-quality in-home nursing care and skilled interventions
- Complete patient assessments and OASIS documentation promptly and accurately
- Collaborate with interdisciplinary care teams for individualized treatment planning
- Provide wound care, medication management, patient education, and disease monitoring
- Communicate important patient status changes to the clinical team
