A reputable healthcare provider is seeking a dedicated Registered Nurse (RN) with home health experience for a 13-week full-time contract in Boston, MA. This role is ideal for motivated RNs with strong clinical skills, OASIS expertise, and a commitment to exceptional patient care in the home setting.
Schedule & Hours:
- Full-time, Monday through Friday
- Potential for on-call responsibilities
Key Responsibilities:
- Deliver comprehensive in-home nursing care and skilled interventions
- Accurately complete OASIS assessments and documentation within required timeframes
- Collaborate effectively with physicians and interdisciplinary teams to develop personalized patient care plans
- Perform skilled tasks including wound care, medication management, patient education, and chronic disease monitoring
- Clearly communicate patient status updates and changes to the clinical team
Qualifications:
- Current, active RN license
- At least one year of recent home health nursing experience
- Demonstrated OASIS proficiency (required)
- HomeCare HomeBase software experience strongly preferred
- Strong organizational skills to handle structured daily schedules and patient loads
- Reliable transportation and a valid drivers license
