Job Description
Provide skilled nursing care to patients in a home health setting, delivering compassionate and comprehensive support to individuals and their families. This contract position requires travel to patient locations as assigned, with weekend rotations approximately once every six weeks.
Key Responsibilities:
- Conduct thorough assessments of patient health conditions and needs.
- Develop and implement personalized nursing care plans.
- Educate patients and families on disease management, medication administration, and health maintenance.
- Complete admission processes accurately and efficiently.
- Manage cases in alignment with organizational standards and community health best practices.
- Maintain timely electronic documentation and facilitate communication within the interdisciplinary care team.
Qualifications & Experience:
- Current RN licensure.
- Minimum of 1 year certified home care nursing experience.
- At least 1 year of case management experience.
- Ability and willingness to travel to patient homes as assigned.
- Strong organizational and communication skills.
- Commitment to patient-centered care and adherence to clinical standards.
