Job Description
We are seeking a dedicated and experienced Registered Nurse with a background in Home Health to deliver high-quality clinical services to patients in their homes. This contract role requires compassionate professionals who thrive in an independent, flexible environment and are committed to providing exemplary patient care across the lifespan.
Schedule/Hours:
- Weekend rotations required, occurring once every 6 weeks
- Must be available for weekend, holiday, and on-call coverage as per department scheduling
Qualifications:
- Active Registered Nurse license in Massachusetts
- Minimum of 1 year of certified home care nursing experience
- At least 1 year of case management experience
- Reliable, independent transportation to travel for home visits
- Willingness to work weekends, holidays, and participate in on-call rotations
- Strong clinical assessment, patient education, and organizational skills
- Familiarity with electronic documentation and interdisciplinary team collaboration
Key Responsibilities:
- Deliver direct clinical care to a diverse patient population (from newborns to seniors) within the home setting
- Perform thorough assessments and collaborate with patients and families to develop, implement, and revise individualized care plans
- Manage the admissions process and ongoing case management duties in line with organizational protocols and regulatory standards
- Provide patient and family education to support ongoing health and wellness
- Ensure precise and timely electronic documentation and maintain clear communication with interdisciplinary team members
