Job Description
Deliver specialized nursing care and meaningful support to patients in their homes, empowering recovery and independence through compassionate, evidence-based practice. This contract opportunity offers RNs the chance to utilize their clinical and case management expertise while collaborating with a multidisciplinary team to ensure optimal patient outcomes in the community setting.
Schedule/Hours:
- Weekend rotations are required once every 6 weeks.
Qualifications:
- Current Registered Nurse (RN) licensure.
- Minimum 1 year of certified Home Care experience.
- At least 1 year of Case Management experience.
- Valid drivers license and reliable transportation; ability to travel between patient visits as assigned.
- Strong organizational, communication, and time management skills.
- Proficiency with electronic health records and timely documentation.
Key Responsibilities:
- Complete comprehensive patient assessments in the home environment and develop individualized care plans.
- Provide skilled nursing care, manage treatments, and monitor progress, adjusting plans as needed.
- Educate patients and families on diagnoses, medications, and self-care strategies to enhance independence and prevent hospital readmissions.
- Coordinate interdisciplinary care, facilitating communication with physicians, therapists, and other healthcare professionals.
- Perform admissions and maintain thorough, accurate electronic documentation in keeping with regulatory guidelines and organizational standards.
- Uphold patient safety, privacy, and satisfaction while working within community health practice standards.
- Participate in periodic weekend rotations as scheduled, ensuring continuity of quality care.
