This 13-week, full-time contract is designed for those who thrive in autonomous work environments and are eager to refine their leadership by working closely with interdisciplinary teams and tackling real-world challenges in patient homes.
Schedule/Hours:
- Monday Friday, 8:00 AM 5:00 PM
Role Highlights:
- Daily travel to patient homes within your assigned territory (mileage reimbursement provided)
- Caseload expectation: 30 points per week (typically 35 patient visits daily)
- Utilize Home Care Home Base (HCHB) for documentation and OASIS assessment tools
Key Responsibilities:
- Perform comprehensive OASIS assessments and develop tailored care plans
- Deliver skilled nursing care including wound management, medication administration, IV therapy, and chronic disease education
- Monitor and evaluate patient progress, responding rapidly to any changes in condition
- Collaborate closely with PTs, OTs, social workers, and home health aides for integrated care delivery
- Facilitate patient and family education on health maintenance and recovery
- Maintain accurate documentation in compliance with regulations and standards
- Participate in ongoing education, team meetings, and case conferences
Qualifications:
- Active RN license (in good standing) for the state of practice
- Minimum 1 year of recent home health field experience
- Proficiency with Home Care Home Base (HCHB) and OASIS required
- Reliable transportation, valid drivers license, and auto insurance

$2,057 - $2,176/Week Estimated weekly earnings represent hourly wages, based on 40 hours per week, and expense reimbursement for eligible candidates. The actual amount may vary based on hours worked, eligibility for reimbursements, and seasonal adjustments.
