Step into an impactful nursing role supporting patients in their homes as part of a dedicated home health care team. This full-time, 13-week contract position is ideal for a registered nurse skilled in patient assessment, case management, and clinical care delivery outside of traditional healthcare settings.

Schedule:

• Monday Friday, 8:00 AM 5:00 PM

Key Responsibilities:

• Conduct OASIS assessments and develop individualized care plans to guide patient care in the home setting.
• Deliver skilled nursing services, including wound care, medication administration, intravenous therapy, and chronic disease management.
• Monitor patient progress, quickly identify changes in health status, and communicate findings to the interdisciplinary care team and physicians.
• Provide education to patients and their families on disease management, medications, and treatments to promote healthy living and prevent complications.
• Complete timely, accurate documentation through the Home Care Home Base (HCHB) system.
• Collaborate closely with physical and occupational therapists, medical social workers, and home health aides to ensure holistic, patient-centered care.
• Participate in regular case conferences, team meetings, and ongoing professional development.
• Maintain compliance with regulatory requirements and agency standards for care and documentation.
• Travel daily within the assigned territory to deliver care (mileage reimbursement provided).

Qualifications:

• Current, unrestricted RN license for the state of practice.
• At least 1 year of recent field-based Home Health experience.
• Proficient in Home Care Home Base (HCHB) electronic documentation system.
• Demonstrated experience with OASIS assessments.
• Reliable transportation, valid drivers license, and auto insurance.

$2,057 - $2,176/Week Estimated weekly earnings represent hourly wages, based on 40 hours per week, and expense reimbursement for eligible candidates. The actual amount may vary based on hours worked, eligibility for reimbursements, and seasonal adjustments.
