Bring your clinical skills to a field-based nursing role providing essential care to patients in their homes over a 13-week, full-time assignment. This position offers a consistent Monday through Friday schedule from 8:00 AM to 5:00 PM, allowing for work-life balance in a rewarding patient-centered environment. Each week, youll maintain a caseload of about 30 pointstypically making three to five patient visits per dayserving individuals throughout your assigned territory with daily travel (mileage reimbursement provided).

Qualifications:

• Active RN license in good standing for the state of practice
• At least one year of recent Home Health experience (field experience strongly preferred)
• Demonstrated proficiency in Home Care Home Base (HCHB)
• Experience with OASIS assessments
• Reliable transportation, valid drivers license, and current auto insurance

Key Responsibilities:

• Conduct thorough OASIS assessments and develop individualized care plans for patients
• Deliver high-quality nursing care, including wound management, medication administration, IV therapy, and management of chronic illnesses
• Monitor patient status, identify any changes, and communicate promptly with care teams and physicians
• Educate patients and families on treatment regimens, medications, and preventive strategies
• Ensure accurate and timely documentation in HCHB software
• Collaborate with an interdisciplinary teamincluding therapists, social workers, and aidesto optimize patient outcomes
• Attend case conferences, staff meetings, and participate in ongoing professional development
• Comply with regulatory and agency care and documentation standards

$2,057 - $2,176/Week Estimated weekly earnings represent hourly wages, based on 40 hours per week, and expense reimbursement for eligible candidates. The actual amount may vary based on hours worked, eligibility for reimbursements, and seasonal adjustments.
