Deliver advanced patient-centered care in a rewarding field-based position caring for individuals in their own homes. This full-time, 13-week contract offers the opportunity to make a significant impact on patients health and comfort while working independently across the community.

Schedule & Setting:

• Monday Friday, 8:00 AM 5:00 PM
• Field-based: visit patient homes within an assigned territory
• Expect 35 patient visits per day (caseload of approximately 30 points per week)
• Daily travel required (mileage reimbursement provided)

Your Responsibilities:

• Conduct comprehensive OASIS assessments and develop tailored care plans for each patient
• Provide skilled nursing care, including wound care, IV therapy, medication administration, and chronic disease management
• Monitor patient progress, report condition changes, and coordinate with the broader care team and physicians
• Educate patients and families regarding disease processes, medication usage, and preventive care measures
• Utilize Home Care Home Base (HCHB) for timely and accurate documentation
• Collaborate with an interdisciplinary team, including therapists and home health aides, to deliver coordinated, high-quality care
• Participate in team meetings, case conferences, and professional development opportunities
• Maintain compliance with agency and regulatory documentation and care standards

Qualifications:

• Active RN license in good standing in the state of practice
• At least 1 year of recent Home Health nursing experience (field work strongly preferred)
• Proficient in Home Care Home Base (HCHB) and experienced with OASIS assessments
• Reliable transportation, valid drivers license, and current auto insurance required

$2,057 - $2,176/Week Estimated weekly earnings represent hourly wages, based on 40 hours per week, and expense reimbursement for eligible candidates. The actual amount may vary based on hours worked, eligibility for reimbursements, and seasonal adjustments.
