Deliver exceptional patient-centered care in a field-based nursing role focusing on home health services. This full-time, 13-week contract involves providing skilled nursing care to patients in their homes, utilizing your expertise with OASIS and the Home Care Home Base (HCHB) documentation system.

Schedule:

• Monday Friday, 8:00 AM 5:00 PM

Key Details:

• Caseload of 30 points per week (typically 35 patient visits daily)
• Daily territory travel with mileage reimbursement

Role Responsibilities:

• Perform thorough patient OASIS assessments and develop care plans tailored to individual needs.
• Deliver skilled nursing care, including IV therapy, wound care, medication administration, and chronic disease management.
• Monitor and document patient progress, recognizing changes in condition and collaborating closely with physicians and interdisciplinary care teams.
• Educate patients and their families on diseases, medications, treatment plans, and preventive strategies.
• Maintain comprehensive, timely, and accurate records using HCHB.
• Work collaboratively with physical and occupational therapists, social workers, and home health aides to support optimal patient outcomes.
• Participate in case conferences, staff meetings, and ongoing education sessions.
• Comply with all regulatory requirements and agency best practices.

Candidate Qualifications:

• Valid RN License in the state of practice, in good standing
• At least one year of recent home health nursing experience in the field
• Proficiency with Home Care Home Base (HCHB) and OASIS assessments required
• Reliable transportation, active drivers license, and current auto insurance
