A dedicated Registered Nurse (RN) with OASIS experience is needed for a full-time, 13-week home health contract. This role is ideal for professionals who excel in clinical documentation, patient management, and enjoy working independently while making an impact in their community.
Schedule:
- 13-week contract with the possibility for extension
- Full-time hours: Monday through Friday
- On-call responsibilities are probable
- Daily/weekly patient visit expectations
Key Responsibilities:
- Deliver in-home nursing care and skilled clinical interventions
- Perform patient assessments with timely, accurate OASIS documentation
- Collaborate with physicians and multi-disciplinary care teams to develop and implement individualized care plans
- Provide wound care, medication administration, patient and caregiver education, and disease management
- Maintain ongoing communication regarding patient status or changes in condition with the clinical team
Qualifications:
- Current, active RN license required
- Minimum of one year recent experience as a home health RN
- Proficient in OASIS documentation
- Experience with HomeCare HomeBase is strongly preferred
- Proven ability to manage a structured, full-time schedule with daily visits
- Reliable transportation and a valid drivers license
