Schedule:

• 13-week contract with full-time hours, Monday through Friday
• Daily patient visits with probable on-call rotation

What You'll Do:

• Provide comprehensive in-home nursing care and skilled interventions
• Complete thorough patient assessments, ensuring accurate and timely OASIS documentation
• Collaborate closely with physicians and multidisciplinary care teams to create and revise care plans
• Perform wound care, administer medications, offer patient education, and conduct disease monitoring
• Communicate effectively with your clinical team about patient status and any changes in condition

Qualifications:

• Active RN license
• At least 1 year of recent home health RN experience
• Proven OASIS documentation skills
• Proficiency with HomeCare HomeBase (strongly preferred)
• Dependable transportation and a valid driver's license
• Ability to handle a set full-time schedule and daily patient visits
