A rewarding opening is available for a Registered Nurse (RN) experienced in home health and OASIS documentation. This 13-week, full-time contract offers the chance to deliver essential care to patients in their homes, making a direct impact on their health and well-being. Schedule:
- 13-week contract with full-time hours, Monday through Friday
- Daily patient visits with probable on-call rotation
What You'll Do:
- Provide comprehensive in-home nursing care and skilled interventions
- Complete thorough patient assessments, ensuring accurate and timely OASIS documentation
- Collaborate closely with physicians and multidisciplinary care teams to create and revise care plans
- Perform wound care, administer medications, offer patient education, and conduct disease monitoring
- Communicate effectively with your clinical team about patient status and any changes in condition
Qualifications:
- Active RN license
- At least 1 year of recent home health RN experience
- Proven OASIS documentation skills
- Proficiency with HomeCare HomeBase (strongly preferred)
- Dependable transportation and a valid driver's license
- Ability to handle a set full-time schedule and daily patient visits
