Your next nursing opportunity starts here. We are seeking a dedicated Registered Nurse with strong home health experience and OASIS proficiency for a rewarding 13-week full-time contract assignment. This position is ideal for an RN who excels at autonomous clinical care, meticulous documentation, and managing a structured schedule while providing compassionate, individualized support to patients in their own homes.
Schedule & Duration:
- Full-time, Monday through Friday
- Occasional on-call responsibilities
- 13-week contract, with a possibility for extension
Key Responsibilities:
- Deliver skilled in-home nursing care and clinical interventions
- Conduct thorough patient assessments and accurately complete OASIS documentation
- Collaborate with physicians and multidisciplinary teams to develop and implement personalized care plans
- Provide wound care, medication management, patient/caregiver education, and chronic disease monitoring
- Ensure timely communication of patient status changes to the clinical team
Qualifications:
- Active and unencumbered RN license
- A minimum of 1 year of recent home health RN experience
- Demonstrated OASIS competency is required
- Proficiency in HomeCare HomeBase is strongly preferred
- Organized and able to manage a busy daily patient schedule
- Reliable transportation and a valid drivers license
