Your next career move starts here! We're seeking a dedicated Registered Nurse (RN) with proven Home Health and OASIS experience for a full-time, 13-week contract assignment. This position is ideal for clinicians who deliver accurate documentation, provide compassionate patient care, and manage a structured daily visit schedule.
Schedule:
- Full-time, Monday through Friday
- On-call duties may be included
Key Responsibilities:
- Deliver in-home nursing care, including skilled interventions
- Accurately perform patient assessments and complete OASIS documentation on schedule
- Collaborate with physicians and multidisciplinary care teams to create and implement individualized care plans
- Conduct wound care, medication administration, patient education, and disease management monitoring
- Keep the clinical team informed of patient status updates and changes in condition
Qualifications:
- Active RN license
- At least 1 year of recent experience as a Home Health RN
- Strong proficiency with OASIS documentation (required)
- Experience using HomeCare HomeBase (strongly preferred)
- Reliable transportation and valid drivers license
- Comfortable working a structured, full-time schedule with daily patient visits
