We are seeking a committed Registered Nurse (RN) with strong OASIS experience for a 13-week, full-time contract. Schedule:

• Full-time, Monday through Friday
• On-call duties may be required

What youll do:

• Provide skilled in-home nursing care and clinical interventions
• Conduct comprehensive patient assessments and complete OASIS documentation efficiently and accurately
• Collaborate with physicians and multidisciplinary healthcare teams to develop and execute individualized care plans
• Oversee wound care, medication management, patient education, and monitor chronic conditions
• Report changes in patient condition promptly to care teams

Requirements:

• Active RN license
• Minimum of 1 year of recent home health RN experience
• Demonstrated experience with OASIS documentation
• Proficiency with HomeCare HomeBase (strongly preferred)
• Ability to manage a structured daily caseload
• Reliable transportation and a valid drivers license
