Schedule:

• Monday Friday, 8:00 AM 5:00 PM

As an RN Home Health nurse, youll manage a caseload expectation of 30 points per week, typically equating to 35 patient visits daily. Your responsibilities center around comprehensive care, utilizing your skills to assess, plan, implement, and evaluate each patients individualized needs in the comfort of their home. Youll leverage technologyincluding the Home Care Home Base (HCHB) electronic documentation system and OASIS assessment toolto ensure thorough and accurate documentation.

Desired Qualifications:

• Active, unencumbered RN license in the state of practice
• At least 1 year of recent experience in Home Health (field experience strongly preferred)
• Proficiency with the HCHB platform and OASIS documentation
• Reliable transportation, current drivers license, and auto insurance

Key Responsibilities:

• Conduct comprehensive OASIS assessments and develop tailored care plans
• Deliver skilled nursing services such as wound care, medication administration, and IV therapy
• Monitor patient progress and coordinate with physicians and the wider care team
• Educate patients and families on disease management, medications, and self-care
• Maintain timely and accurate records using HCHB
• Collaborate closely with physical therapists, occupational therapists, social workers, and aides
• Attend case conferences, staff meetings, and continuing education sessions

$2,057 - $2,176/Week Estimated weekly earnings represent hourly wages, based on 40 hours per week, and expense reimbursement for eligible candidates. The actual amount may vary based on hours worked, eligibility for reimbursements, and seasonal adjustments.
