Responsibilities include:
- Providing in-home nursing care and skilled clinical interventions
- Performing patient assessments and delivering comprehensive, timely OASIS documentation
- Collaborating with physicians and interdisciplinary care teams to tailor care plans
- Administering wound care, managing medications, and offering patient/caregiver education
- Monitoring disease progression and updating the clinical team on changes in condition
Qualifications:
- Active RN license
- Minimum 1 year of recent home health RN experience
- Demonstrated proficiency with OASIS documentation (required)
- Experience with HomeCare HomeBase (strongly preferred)
- Strong organizational and time management skills for daily visit schedules
- Reliable transportation and valid drivers license
