Schedule:
- Full-time, Monday through Friday
- On-call coverage likely required
Key Responsibilities:
- Deliver skilled in-home nursing care and clinical interventions
- Complete comprehensive patient assessments with timely, thorough OASIS documentation
- Develop and coordinate individualized care plans in collaboration with physicians and interdisciplinary teams
- Provide wound care, medication management, patient and family education, and disease monitoring
- Clearly communicate updates and any changes in patient condition to the broader care team
Required Qualifications:
- Active and unrestricted RN license
- At least 1 year of recent home health RN experience
- OASIS documentation experience required
- Proficiency with HomeCare HomeBase software strongly preferred
- Reliable transportation and a valid drivers license
- Ability to independently manage a structured schedule, including daily patient visits
