Schedule:
- Full-time, Monday through Friday
- On-call duties are probable
Key Responsibilities:
- Deliver direct home health nursing care and interventions to patients
- Complete thorough assessments and OASIS documentation promptly and accurately
- Collaborate with physicians and interdisciplinary teams to create personalized care plans
- Provide wound care, medication management, disease monitoring, and patient education
- Maintain open, timely communication regarding changes in patient condition
Desired Qualifications:
- Active Registered Nurse (RN) license
- At least one year of recent home health RN experience
- OASIS documentation proficiency is required
- Familiarity with HomeCare HomeBase strongly preferred
- Dependable transportation and a valid drivers license
- Excellent communication and time management skills
