Job Description

An experienced Registered Nurse (RN) with a strong home health background is needed to provide high-quality patient care on a contract basis. This role requires expertise in OASIS to ensure accurate patient assessments and care planning within patients' homes.

Key Responsibilities:

  • Deliver comprehensive nursing care, including vital sign monitoring, medication management, and wound care.
  • Conduct initial and ongoing patient assessments and update care plans accordingly.
  • Educate patients and families on disease management, treatment plans, and preventive measures.
  • Collaborate closely with physicians, therapists, and other healthcare professionals to coordinate care.
  • Maintain accurate and timely documentation of all patient care activities in electronic medical records (EMR).
  • Monitor patient conditions and communicate any changes to the supervising physician.
  • Ensure compliance with all applicable state and federal home health regulations.

Qualifications:

  • Active Registered Nurse (RN) license.
  • Minimum of 3 months home health experience utilizing OASIS.
  • Experience with HCHB is highly desirable.
  • Strong clinical skills in IV therapy, wound care, and medication management.
  • Proficiency with EMR systems.
  • Excellent communication and interpersonal abilities.
  • Self-motivated with effective time management skills.
  • Valid drivers license and reliable transportation required.
