Job Description

Deliver compassionate, individualized care as a registered nurse in a home health setting. This full-time, 13-week contract role provides a meaningful opportunity to work directly with patients in their homes, helping to improve health outcomes and quality of life.

Schedule & Hours:

  • Monday Friday, 8:00 AM 5:00 PM

Key Responsibilities:

  • Conduct comprehensive OASIS assessments and develop personalized care plans.
  • Deliver skilled nursing care, including wound care, IV therapy, medication administration, and chronic disease management.
  • Monitor patient progression, identify changes in condition, and maintain prompt communication with physicians and the care team.
  • Educate patients and family members about disease processes, medications, treatment regimens, and preventive strategies.
  • Maintain accurate, timely electronic documentation via Home Care Home Base (HCHB).
  • Collaborate effectively with physical therapists, occupational therapists, social workers, and aides.
  • Participate actively in case conferences, staff meetings, and continuing education opportunities.
  • Comply with all regulatory standards for documentation and patient care delivery.

Qualifications:

  • Current, active RN license in good standing for the state of practice.
  • At least 1 year of recent home health nursing experience (field work highly preferred).
  • Proficiency with Home Care Home Base (HCHB) and OASIS required.
  • Reliable transportation, valid drivers license, and active auto insurance.

$2,057 - $2,176/Week Estimated weekly earnings represent hourly wages, based on 40 hours per week, and expense reimbursement for eligible candidates. The actual amount may vary based on hours worked, eligibility for reimbursements, and seasonal adjustments.
