Schedule/Hours:

• Travel-based home visits within the greater Baltimore area
• Periodic in-person office meetings

Qualifications:

• Active, unencumbered RN license in Maryland (or eligible via Nurse Licensure Compact)
• Minimum of 2-3 years nursing experience, with prior home health preferred
• Current CPR certification
• Graduate of an accredited nursing program (BSN preferred)
• Strong assessment skills and familiarity with age-specific patient requirements
• Proficiency with computer skills, including secure documentation and online training
• Reliable transportation, current driver's license, and required liability insurance

Responsibilities:

• Deliver and coordinate patient-centered home nursing care following physician orders and established protocols
• Act as primary RN, supervising and evaluating Home Health Aides and LPNs/LVNs as required by state and agency standards
• Conduct thorough assessments, develop individualized care plans, and revise as patients needs evolve
• Maintain accurate patient records, promptly report significant changes to the physician, and communicate updates to all care team members
• Arrange appropriate counseling for patients and families and participate in staff education initiatives
• Ensure compliance with all relevant federal and state regulations
• Regularly attend meetings and participate in ongoing professional development
