A full-time, 13-week assignment is available for Registered Nurses specializing in Home Health. This role is field-based, serving patients in their homes and supporting recovery and independence. Nurses will follow a Monday through Friday, 8:00 AM to 5:00 PM schedule, managing a caseload averaging 35 patient visits daily (30 points per week). The position includes daily travel within an assigned territory, with mileage reimbursement provided.

Key Responsibilities:

• Conduct comprehensive OASIS assessments and develop individualized patient care plans.
• Deliver skilled nursing care, including wound care, medication administration, IV therapy, and management of chronic diseases.
• Regularly evaluate patient progress; promptly identify and report changes in condition to physicians and care team members.
• Educate patients and families on treatments, disease management, medications, and health promotion.
• Document all care accurately and timely in Home Care Home Base (HCHB).
• Collaborate closely with physical therapists, occupational therapists, medical social workers, and home health aides.
• Attend case conferences, staff meetings, and continuing education sessions.
• Consistently adhere to agency and regulatory standards for patient care and documentation.

Desired Qualifications:

• Active RN license (in good standing) for the state of practice.
• At least 1 year of recent, hands-on home health experience is required.
• Demonstrated proficiency in Home Care Home Base (HCHB) and OASIS.
• Reliable transportation, a valid drivers license, and auto insurance are essential.

$2,057 - $2,176/Week Estimated weekly earnings represent hourly wages, based on 40 hours per week, and expense reimbursement for eligible candidates. The actual amount may vary based on hours worked, eligibility for reimbursements, and seasonal adjustments.
