Your next nursing opportunity starts here with a rewarding role as a Registered Nurse in home health care. We are seeking a skilled RN with strong OASIS documentation experience for a 13-week full-time contract. This position offers a structured schedule, professional growth, and a chance to deliver high-impact care to patients in their homes.
Schedule:
- 13-week contract with the possibility of extension
- Full-time hours, Monday through Friday
- On-call responsibilities may be required
Key Responsibilities:
- Deliver skilled nursing care and clinical interventions in patients homes
- Accurately complete OASIS assessments and maintain timely documentation
- Collaborate closely with physicians and interdisciplinary care teams to implement individualized care plans
- Provide wound care, medication administration, disease management, and comprehensive patient education
- Communicate patient condition changes promptly to the clinical team
Qualifications:
- Current, active RN license
- At least one year of recent experience as a home health RN
- Proficient in OASIS documentation (required)
- Familiarity with HomeCare HomeBase (highly preferred)
- Organized and able to manage a full-time home visit schedule independently
- Reliable transportation and a valid drivers license
