Your next nursing opportunity starts here. We are searching for a dedicated Registered Nurse with OASIS experience to take on a 13-week full-time home health contract. This position is ideal for nurses with strong clinical skills, attention to documentation, and the ability to efficiently manage a structured caseload.

Schedule:

• Full-time, Monday through Friday
• On-call responsibilities may be included

Key Responsibilities:

• Deliver skilled in-home nursing care and patient interventions
• Perform thorough patient assessments and complete OASIS documentation promptly and accurately
• Partner with physicians and interdisciplinary care teams to implement individualized care plans
• Provide wound care, medication administration, patient education, and health status monitoring
• Communicate patient updates and any changes in condition with the clinical team

Desired Skills and Experience:

• Active RN license required
• Minimum of one year of recent experience in home health as an RN
• OASIS proficiency is a must
• Experience with HomeCare HomeBase is strongly preferred
• Strong organization and time management skills for daily patient visits
• Reliable transportation and a valid drivers license
