Make a meaningful impact in patient lives by delivering skilled care where it matters mostat home. This full-time, 13-week assignment offers the opportunity to work independently while remaining part of an interdisciplinary team dedicated to quality outcomes in a field-based setting.

Schedule & Work Environment

• Monday Friday, 8:00 AM 5:00 PM
• Daily travel within your assigned territory (mileage reimbursed)
• Manage a caseload expectation of 30 points weekly (35 home visits per day)
• Utilize the Home Care Home Base (HCHB) electronic documentation system

Core Responsibilities

• Conduct comprehensive OASIS assessments and develop individualized care plans
• Deliver skilled nursing care, including wound care, IV therapy, medication administration, and chronic disease management
• Monitor patient progress, recognize changes in condition, and communicate effectively with the care team and physicians
• Educate patients and families on conditions, ongoing treatments, medications, and preventive care measures
• Accurately document all care and interventions in HCHB in a timely manner
• Collaborate closely with physical/occupational therapists, social workers, and home health aides
• Participate in interdisciplinary case conferences, staff meetings, and professional development
• Uphold all clinical and documentation standards per regulatory and agency guidelines

Qualifications Sought

• Active RN license in good standing in the state of practice
• At least one year of recent Home Health field experience
• Experienced with Home Care Home Base (HCHB) and OASIS assessments
• Reliable transportation, current drivers license, and auto insurance

$2,057 - $2,176/Week Estimated weekly earnings represent hourly wages, based on 40 hours per week, and expense reimbursement for eligible candidates. The actual amount may vary based on hours worked, eligibility for reimbursements, and seasonal adjustments.
