Take the next step in your nursing career by supporting patients in a home health setting through a full-time, 13-week contract opportunity. As a Registered Nurse specializing in home health, you will work with patients in their homes, delivering compassionate care and promoting positive outcomes.

Schedule/Hours:

• Monday Friday, 8:00 AM 5:00 PM
• Full-time, 13-week contract

Key Responsibilities:

• Conduct comprehensive OASIS assessments and formulate individualized care plans.
• Deliver skilled nursing care, such as wound care, medication administration, IV therapy, and management of chronic conditions.
• Monitor patient progress, recognizing changes and communicating with the healthcare team and physicians as needed.
• Provide patient and family education on disease processes, treatment plans, medications, and preventive care.
• Maintain accurate and timely documentation in the Home Care Home Base (HCHB) system.
• Collaborate with physical therapists, occupational therapists, medical social workers, and home health aides for coordinated patient care.
• Attend case conferences, staff meetings, and maintain ongoing professional development.
• Adhere to all regulatory and agency standards for documentation and care delivery.
• Travel daily to visit patients within your assigned territory (mileage reimbursement provided).

Qualifications:

• Current, active RN license (state of practice) in good standing
• Minimum 1 year of recent Home Health experience (field experience highly preferred)
• Proficiency with Home Care Home Base (HCHB) and OASIS documentation required
• Reliable transportation, valid drivers license, and current auto insurance

$1,815 - $1,920/Week Estimated weekly earnings represent hourly wages, based on 40 hours per week, and expense reimbursement for eligible candidates. The actual amount may vary based on hours worked, eligibility for reimbursements, and seasonal adjustments.
