We are seeking a dedicated Registered Nurse (RN) with OASIS experience to deliver high-quality care in a rewarding home health environment. This full-time, 13-week contract offers you the chance to demonstrate your clinical expertise, ensure precise documentation, and work a structured schedule in a supportive, professional setting.
Schedule:
- 13-week contract with extension potential
- Full-time, Monday through Friday
- On-call participation may be required
- Structured daily patient visits
Key Responsibilities:
- Deliver in-home nursing care and skilled interventions
- Complete thorough patient assessments and timely, accurate OASIS documentation
- Collaborate with physicians and interprofessional teams on individualized care plans
- Conduct wound care, medication management, patient education, and disease process monitoring
- Keep the clinical team informed with regular updates and any changes in patient status
Qualifications:
- Active RN license
- At least 1 year of recent home health RN experience
- Proficiency with OASIS is mandatory
- Familiarity with HomeCare HomeBase preferred
- Comfortable with managing a full-time schedule of daily visits
- Reliable transportation and valid drivers license
