Your next opportunity in nursing awaits. Were seeking a dedicated Registered Nurse with home health experience to join a welcoming care team for a 13-week full-time assignment. This role focuses on delivering high-quality, patient-centered care right in the comfort of clients homes, emphasizing exceptional documentation and autonomy.
Schedule:
- 13-week contract with possible extension
- Full-time, Monday through Friday
- On-call responsibilities may apply
Essential Qualifications:
- Active RN license
- At least one year of recent home health experience (required)
- Strong OASIS proficiency required for timely and accurate documentation
- Experience with HomeCare HomeBase EMR (strongly preferred)
- Ability to manage a structured daily visit schedule
- Reliable transportation and a valid drivers license
Primary Responsibilities:
- Deliver skilled nursing care and individualized interventions in patients homes
- Perform comprehensive patient assessments and document findings using OASIS
- Collaborate with physicians and interdisciplinary teams to construct and manage tailored care plans
- Provide wound care, manage medications, and educate patients and families on disease processes and self-care
- Monitor patients conditions and provide timely updates to the clinical team
