Experienced registered nurses passionate about providing high-quality care in a home health setting will thrive in this dynamic, field-based opportunity. This full-time, 13-week contract position offers the chance to make a meaningful impact by supporting patients in the comfort of their own homes.
Schedule:
- Monday to Friday, 8:00 AM 5:00 PM
Key Qualifications:
- Current and unrestricted RN license for the state of practice
- At least one year of recent home health nursing experience (field work preferred)
- Solid knowledge and proficiency with Home Care Home Base (HCHB) for documentation
- Extensive experience conducting OASIS assessments
- Dependable transportation, valid drivers license, and current auto insurance
Primary Responsibilities:
- Perform thorough OASIS assessments in patient homes and develop personalized care plans
- Deliver skilled nursing services, from wound and medication management to IV therapy and chronic disease support
- Monitor patient status, document changes, and communicate promptly with the interdisciplinary care team
- Educate patients and families about health conditions, medications, and preventive care
- Ensure accurate and timely documentation in HCHB
- Collaborate with therapists and social workers to coordinate holistic care
- Participate in meetings, case conferences, and ongoing training
- Comply with all applicable regulatory and agency standards

$1,790 - $1,958/Week Estimated weekly earnings represent hourly wages, based on 36 hours per week, and expense reimbursement for eligible candidates. The actual amount may vary based on hours worked, eligibility for reimbursements, and seasonal adjustments.
