Schedule:
- Full-time, Monday through Friday
- On-call rotations may be required
- Structured daily patient visits
Key Responsibilities:
- Deliver skilled in-home nursing care, including assessments, wound care, and medication management
- Complete OASIS documentation thoroughly and punctually for each patient
- Collaborate closely with physicians and interdisciplinary team members to develop and implement tailored care plans
- Educate patients and caregivers, monitor chronic conditions, and respond promptly to changes in status
- Communicate important updates to the care team, ensuring seamless patient transitions and quality outcomes
Qualifications:
- Active RN license in good standing
- At least 1 year of recent home health RN experience
- Demonstrated experience with OASIS documentation (required)
- Proficiency in HomeCare HomeBase documentation platform (strongly preferred)
- Able to confidently manage full-time schedules and daily travel between patient homes
- Reliable transportation and valid drivers license
