Job Description
Deliver essential patient care in a dynamic home health environment as part of a dedicated nursing team. This full-time, 13-week contract position is structured to serve patients in a field-based capacity, visiting their homes daily within the assigned territory.
Schedule/Hours:
- Monday through Friday, 8:00 AM 5:00 PM
- Expect 35 patient visits per day (targeting a 30-point weekly caseload)
Qualifications:
- Active and unencumbered RN license in the state of practice
- At least one year of recent experience in Home Health, preferably field-based
- Proficiency with Home Care Home Base (HCHB) electronic documentation
- Demonstrated experience with OASIS assessments
- Reliable vehicle, valid driver's license, and current auto insurance
Key Responsibilities:
- Complete comprehensive OASIS assessments and develop individualized care plans
- Deliver skilled nursing care including wound care, IV therapy, medication administration, and management of chronic illnesses
- Monitor patient progress and promptly report changes to the multidisciplinary care team and physicians
- Educate patients and families on care plans, medication usage, and proactive health measures
- Accurately document all care using Home Care Home Base (HCHB)
- Collaborate closely with therapists, medical social workers, and home health aides to ensure patient-centered, coordinated care
- Attend case conferences, staff meetings, and participate in ongoing education
- Maintain compliance with all regulatory and agency standards

$2,057 - $2,176/Week Estimated weekly earnings represent hourly wages, based on 40 hours per week, and expense reimbursement for eligible candidates. The actual amount may vary based on hours worked, eligibility for reimbursements, and seasonal adjustments.
