Job Description

Deliver essential nursing care and support to patients in a field-based Home Health setting through this 13-week, full-time contract opportunity. As an RN, you will engage with the community and provide individualized care to patients in their homes, focusing on health improvement and enhanced quality of life.

Schedule:

  • Monday Friday, 8:00 AM 5:00 PM
  • Daily travel to patient homes within your assigned territory
  • Caseload expectation: 35 patient visits per day (30 points per week)

Key Responsibilities:

  • Perform comprehensive OASIS assessments and develop tailored plans of care
  • Deliver skilled nursing, including wound care, IV therapy, medication administration, and chronic disease management
  • Monitor patient conditions and promptly communicate notable changes to the care team and physicians
  • Educate patients and families on treatment, medication, and preventative care
  • Document visits and interventions accurately and efficiently in Home Care Home Base (HCHB)
  • Coordinate with other disciplines such as PT, OT, medical social workers, and home health aides to ensure seamless care
  • Participate in case conferences, staff meetings, and continuing education
  • Maintain compliance with all regulatory and agency standards

Qualifications:

  • Active RN license in good standing for the state of practice
  • At least one year of recent Home Health nursing experience (field experience strongly preferred)
  • Proficiency with Home Care Home Base (HCHB) and OASIS assessment
  • Reliable transportation, valid drivers license, and current auto insurance

$2,057 - $2,176/Week Estimated weekly earnings represent hourly wages, based on 40 hours per week, and expense reimbursement for eligible candidates. The actual amount may vary based on hours worked, eligibility for reimbursements, and seasonal adjustments.
