Job Description

Embark on a rewarding opportunity providing essential nursing care in the home health setting. This full-time, 13-week contract position offers a stable Monday through Friday schedule, from 8:00 AM to 5:00 PM, allowing you to positively impact patients well-being in their homes while maintaining work-life balance. Daily responsibilities include managing a caseload averaging 35 patient visits per day (approximately 30 points per week) throughout an assigned territory, with daily travel compensated by mileage reimbursement.

Desired Qualifications:

• Active RN license in good standing for the state of practice
• At least one year of recent home health nursing experience, with field experience highly preferred
• Proficiency with Home Care Home Base (HCHB)
• Previous experience completing OASIS assessments
• Reliable transportation, a valid drivers license, and current auto insurance

Key Responsibilities:

• Conduct thorough OASIS assessments and develop individualized care plans for patients
• Deliver skilled nursing services, including wound care, medication administration, IV therapy, and chronic disease management
• Monitor patient progress and promptly communicate changes to the care team and physicians
• Provide patient and family education on conditions, medications, treatment plans, and preventive care
• Collaborate closely with other professionals such as physical therapists, occupational therapists, social workers, and home health aides
• Accurately document all care and communication in Home Care Home Base (HCHB)
• Participate in team meetings, case conferences, and ongoing education
• Remain compliant with regulatory and agency standards for documentation and patient care delivery
