Job Description

Deliver essential care to patients in their homes through a full-time, 13-week assignment supporting individuals recovery and well-being. This field-based opportunity offers a consistent schedule, working Monday through Friday, 8:00 AM to 5:00 PM. Expect a manageable caseload of 35 home visits daily (approximately 30 points per week), serving a defined territory with mileage reimbursement for daily travel.

Key Qualifications:

  • Active RN license (in good standing) in the state of practice
  • At least one year of recent home health experience in a field role
  • Skilled in Home Care Home Base (HCHB) electronic documentation
  • Experienced with OASIS assessment tool
  • Reliable vehicle, current drivers license, and valid auto insurance

Primary Responsibilities:

  • Complete comprehensive OASIS assessments and develop individualized care plans
  • Deliver skilled nursing interventions: wound care, IV therapy, medication administration, and chronic disease management
  • Monitor patient progress, report changes, and coordinate closely with physicians and interdisciplinary team members
  • Educate patients and families on treatment plans, disease management, and preventive health measures
  • Maintain accurate, timely clinical documentation within HCHB
  • Participate in case conferences, continuing education, and team meetings
  • Uphold regulatory and agency standards for quality documentation and care delivery

$1,113 - $1,178/Week Estimated weekly earnings represent hourly wages, based on 40 hours per week, and expense reimbursement for eligible candidates. The actual amount may vary based on hours worked, eligibility for reimbursements, and seasonal adjustments.
