Advance your nursing career by delivering patient-centered care in a dynamic, field-based setting. This full-time, 13-week contract offers the opportunity to make a meaningful impact on patients lives through skilled nursing within their homes.

Schedule:

• Monday Friday, 8:00 AM 5:00 PM

Key Responsibilities:

• Conduct comprehensive OASIS assessments and develop individualized care plans
• Provide skilled nursing services including wound care, medication administration, IV therapy, and chronic disease management
• Evaluate patient progress, promptly communicate any changes with the care team and physicians
• Educate patients and families about disease processes, treatment options, medication management, and preventive care
• Accurately document all interventions and communications using Home Care Home Base (HCHB)
• Collaborate with interdisciplinary team membersphysical therapists, occupational therapists, medical social workers, and home health aidesto coordinate holistic care
• Attend case conferences, participate in staff meetings, and pursue continuing education
• Travel daily to patient homes within your assigned territory (mileage reimbursement provided)
• Adhere rigorously to agency and regulatory standards for care and documentation

Qualifications:

• Active RN license in good standing in the state of practice
• At least one year of recent home health experience (field-based experience strongly preferred)
• Proficiency in OASIS and experience with Home Care Home Base (HCHB) electronic documentation
• Reliable transportation, a valid drivers license, and current auto insurance

$1,113 - $1,178/Week Estimated weekly earnings represent hourly wages, based on 40 hours per week, and expense reimbursement for eligible candidates. The actual amount may vary based on hours worked, eligibility for reimbursements, and seasonal adjustments.
