Job Description

Deliver high-quality nursing care with a dedicated focus on patients in their own homes on a full-time, 13-week contract. This field-based position uses your clinical skills as a Registered Nurse, supporting patient recovery and independence within a supportive professional team.

Schedule & Setting:
Monday Friday, 8:00 AM 5:00 PM
Field-baseddaily travel to patient homes within an assigned territory
Caseload: 30 points per week (typically 35 patient visits per day)

Key Responsibilities:
Conduct thorough OASIS assessments and develop individualized patient care plans
Deliver a range of skilled nursing services, including wound care, medication administration, IV therapy, and chronic disease management
Monitor and evaluate patient progress, noting any changes and communicating promptly with providers and the interdisciplinary team
Educate patients and caregivers about disease management, medication regimens, and wellness strategies
Maintain accurate and timely electronic documentation using Home Care Home Base (HCHB)
Collaborate effectively with physical therapists, occupational therapists, social workers, and home health aides
Participate in case conferences, staff meetings, and continuing education
Uphold all clinical, regulatory, and documentation standards

Qualifications:
Active, unencumbered RN license in Minnesota
Minimum 1 year recent home health field experience
Demonstrated proficiency with Home Care Home Base (HCHB) and OASIS assessments
Reliable transportation, valid drivers license, and active auto insurance

$1,113 - $1,178/Week Estimated weekly earnings represent hourly wages, based on 40 hours per week, and expense reimbursement for eligible candidates. The actual amount may vary based on hours worked, eligibility for reimbursements, and seasonal adjustments.
