A 13-week full-time contract is available for a skilled Registered Nurse (RN) with home health experience and proven OASIS documentation skills. This dynamic role requires a confident clinician with strong assessment, care coordination, and documentation abilities, ready to support patients in their homes with professionalism and compassion.
Schedule:
- Full-time, Monday through Friday
- Daily visit structure with probable on-call responsibilities
Responsibilities include:
- Delivering direct nursing care and clinical interventions to home health patients
- Conducting thorough assessments and completing timely, accurate OASIS documentation
- Collaborating with physicians and interdisciplinary care teams to develop individualized care plans
- Providing wound care, medication management, disease monitoring, and patient/caregiver education
- Communicating any changes in patient condition promptly to the clinical team
Qualifications:
- Active RN license in good standing
- At least one year of recent home health RN experience
- OASIS proficiency (required)
- Experience with HomeCare HomeBase (strongly preferred)
- Reliable transportation and valid drivers license
- Ability to independently manage a consistent, full-time visit schedule

$2,193 - $2,312/Week Estimated weekly earnings represent hourly wages, based on 40 hours per week, and expense reimbursement for eligible candidates. The actual amount may vary based on hours worked, eligibility for reimbursements, and seasonal adjustments.
